EDIT: IVE TRIED UPGRADING POWER SUPPLY TO 750 WATTS.
After playing a game for 3-5 minutes, the game will freeze, then turn black for a few seconds and come back with the game crashed. The last seconds of the audio would be on a loop. Then, I have to restart my computer and can't play any game without it crashing after 5 minutes. It doesn't matter if it's Roblox or Doom it still crashes. They sent me a new one, but the same thing happened and my warranty ran out. Amazon and Cyberpower refused to help. I've contacted AMD, PowerColor, ASUS, Cyberpower, Smart Guard Protection, and even Amazon. No one wants to help. I've been reading support forums and emailing AMD for the past year no luck. When they did send me a new graphics card it worked for a few months. I've tried everything. I know what youre going to say DDU blah blah...Doesn't work. The only thing that worked was lowering my GPU Frequency. Please help... AMD knows this issue is happening and are doing NOTHING about it. In fact, they're losing money on the market and I'm glad. I can't afford this. Please help.

I don't know what a windows reliability history test is and I don't see run a system diagnostic report is. BTW not using drivers works.

Run them/save to file, attach them(the saved files) to next reply, want all 4 of them..
Windows hardware errors could be anything from cpu/ram/hdd/ssd/psu/, corrupt OS/or other corrupt software, bad overclock or overclocking software.
Really need to have the saved report files that were asked for, to see what actual errors are.
Or you take the pc to a local repair/service store (or call rent a geek/geeksquad) pay them to check your whole system.
Or wipe your ssd/hdd clean and reinstall OS and all software.
Or you try posting on the tenforums.com > BSOD And Debugging section (they use an app. that can usually pick all errors).
My card worked well for months then that happened I got a new one and the same thing happened. (Same model)
So what, you want help, supply the info.
Your beginning to look suspect, with postings on old threads, refusing to supply info.
If you do not know how to supply the info, then go to the tenforums site, they can walk you through it all.
what type of info would you like?
In the search box, type, dxdiag , click run, save it.
In your search box, type, msinfo, open it, save it,
Attach the files(as an attachment).
When you open reply, click, Use advanced editor(top right), then you will see the 'Attach' bottom right.
Same issu with R9 380X. It is not your fault.

Mostly see TDR, radeon settings errors.
Start with radeon settings, usually end result of other issue, but can also be caused by conflict of other 3rd party oc/fan control apps. with wattman. Usual solution is use one or the other(not both). reset any/all 3rd party app/s.(uninstall them if you choose to use wattman), also reset wattman to default(Restore Factory Defaults), re-enable wattman. If you choose to use 3rd party app/s.(Do not enable wattman).

Before you try that though, run a defrag on the HDD, then DISM commands(tutorials on tenforums) for some of your OS issues. Might also be an idea to run a HDD test for bad sectors(use the WD software, not windows chkdsk).

When the R9 380X came out, i bought a brand new model. (Sapphire Nitro OC 4gb)
This time, the AMD forum was full of Black screen error topic. Not only the R9 3XX, but almost every card.
The Solution was: go back to catalyst. 15.11 i think that one was not supporting DX12.
So, i tried almost every driver this time. Around 14-16.
Only one worked. That was in 2016.
2017 was almost perfect without any BSOD. I used Clockblocker for all my games and custom fan profile with MSI afterburner.
In 2018 i had only one BS error last week.
I cant use my card anymore. Last night i had a BS. (i was playing RPCS3 emu, crash amdvlk64.dll error)
Since that crash, every driver give me BSOD even on desktop.After a succesfull booting, i had a BSOD but it can reveive itself, saying wattman balblabla restore default.
After restart BSOD started again. I took out my card. It is impossible to use my PC with IT.
Tried, DDU, 18.7.1, 18.5.1, 18.2.1, 17.12. disabled windows update, deleted windows update.
with DVI cable i have BSOD
with Hdmi cable i have a greean screen of death. (funny)
The card is clear, had new thermal paste last month, temps are 60 celsius on load.
Of course i ran out of warrianty in april.... lol

If you have not done so already. Slide your red horizontal "Power Limit" slider all the way to the right to its maximum. It is in Radeon Settings / Gaming / Global Settings / Global Wattman. See if that helps your situation. The symptoms you described are commonly fixed by this. My RX 580 does the same thing at default settings with the power slider. Black screen with audio still playing, in loop. This issue was introduced in the drivers a little over a year ago when they introduced Wattman and it is still an issue. If you had been cruising along with older drivers you would not have experienced this until now.
