You should have already received an email asking you to confirm the shipping address.

In case you got the initial response, just respond that email.
I responded to the initial email with the shipping address but didn't get a response in return
One thing that I found odd is that the message states:
"In order to update this service request, please respond, leaving the service request reference intact."
But the subject of the email was simply: "RE: Warranty Service Request"
Okay, I'm escalating this issue, someone will get back to you in the next 24 hours.
How can we check on the status of a ticket? I also requested a boot kit and got a ticket number but have not had any other emails from AMD.
I had the same problem bout two months ago when I submitted my own form and it didn't get replied to.
The issue is that the body of your reply needs to include the ticket # in it.
They probably parse emails for the # to determine what to do with them, and emails that don't have it just don't get replied to.
