On 1809 with 19.5.1 shows for me.
Could be any combo of 1903/gpu-z/driver not being read correctly.

Problem is in driver initialisation. Vulkan games as Rage 2 works without problems but GPU is unknown. I download Vulkan Hardware Capability Viewer and here is GPU still Unknown AMD GPU.

Rage 2 is Vulkan only so that game will be problematic for many who have issues.
from what ive tested this doesnt give any issues in games though im mostly going off of Doom and Emulators like Dolphin, PPSSPP, and Source Port GZDoom and GZDoom crashes due to read access violations possibly not caused by this but i was told it could be crashing due to the program not knowing what GPU its interfacing with
Doom 2016 seems to work ok with OGL
works fine on Vulkan too even if Vulkan doesnt know the name to its GPU lol
have you upgraded to 19.5.2 yet??
Now on 1903 and see same, suspect this is a gpu-z read issue with 1903.
Might be an idea to report it on their forum.

Maybe the Dev needs to update the Dec.'18 vers. for 1903 ?
Of course also report it to the amd driver team using 'Report Issue Online'.
I report problem 2 times.
Problem remains in 19.6.1.
I have one game that I loaded up and the OSD said VULKAN
Wolfenstein II was such a game
Game was working ok at 3840x2160 when testing games
I have also tested my Vega 11 for several games but it is not as powerful as my GTX 1060 for some demanding games
it's not the driver the problem is from windows 10 1903

There was a smallish update today for 1903 but I suspect more updates are pending to be delivered soon
AMD has been cranking out updates regularly so they are trying to stay on top of things
