R9 380 Constant Green Screen Crashes While Gaming

I've got a R9 380 around 2 years old that's been solid ever since a few days or so ago.  I recently upgraded Windows to 1903 and I'm not sure if that's the cause.  I've been playing Warhammer 40K Inquisitor and Assassins Creed Odyssey and both games green screen within 5 minutes (if I'm lucky) of playtime.

Things I've tried:

- Reinstalled Windows

- Reinstalled GPU drivers a bunch and have tried older drivers

-Updated BIOS

-Set a QWORD for TdrDelay in Computer\HKEY_LOCAL_MACHINE\SYSTEM\CurrentControlSet\Control\GraphicsDrivers

-Tried another PCI slot

-cleared CMOS

-Turned off "Fast Startup" in Power Options

-thoroughly dusted all the guts

I've obviously seen a ton of other posts on this issue, but nothing has yet to resolve the problem for me.  I'm currently downloading a 1809 iso of Windows and may try that.  I attached a dxdiag in hopes something glaringly jumps out to someone.  I don't overclock, but I have used MSI Afterburner to adjust my fan speeds, but that has since been uninstalled.  Could I simply just have a bad GPU after 2 years?
