Im getting this issues on certain games and mostly at distances like in Modern Warfare the smoke has it only and hell let loose it appears in the distance but I'm running a Red Devil RX 5700 XT that i just bought 2 weeks ago.
it's artifacting. If you are running at default clocks and voltages it's possible you have a faulty card. Suggest underclocking GPU core and VRAM to see if it disappears.
