Here the steps to reproduce the problem:
1. Collect a Base Line with Time Spy Extreme. Hot Spot 80°C and fan speed 1700
2. Set Power Target to +15% under manual tuning and apply changes. Then chose the default profile
3. Start a game with a manual power profile and quit the game. I used some manual settings with Battlefield. After the game ends the power profile should be Default
4. Make a new Test with Time Spy Extreme. Hot Spot now 98°C rising and fan speed 2900. Same situation in other games
After a reboot you have the same failure with the hot temperature and high fan speed. The power profile is shown as default so you can't see what's going wrong. If you don't make step 4 the power profile is shown as manual with power target +15%.
With this bug the games run with power target +0 (350W) but with hot temperature and high fan speed.
To fix the problem you can set Power Target to 0% and apply changes and then chose the default profile. The default profile works fine now.
