Strange GPU driver crash

I'm having some long-running issues with newer GPU drivers.

system:
Win10 Pro x64 22H2
Ryzen 7 5800x
Radeon RX 6700 XT
2 displays connected, main runs apps on 120 Hz, secondary runs chrome on 60 Hz


My issue is that drivers after 24.8.1 (this year's august) keep crashing on my system.
What exactly happens is, Im running the game on main monitor (Most commonly happens in Path of Exile, Path of Exile2), and I have some kind of video stream running in chrome on secondary monitor like a youtube video or a twitch stream, and both monitors turn black, the sound stops for 4-5 seconds, sound comes back but monitors stay black and they eventually turn off because no video feed. After this I have to reset the PC, when it reboots I get the Windows popup saying "There are hardware changes, Please restart system" and when I try to open AMD Adrenaline it either won't start at all or gives me an error message saying driver is incompatible with the video card, install the correct driver.

Bandaid fixing includes uninstalling the GPU driver with DDU, rebooting then reinstalling the current up-to-date driver but shortly after (this could happen multiple times a day or not for a week then it returns) it happens again.

I disabled Windows updating drivers in GPO, I disabled automatic driver update and download in the AMD software, to my knowledge there shouldn't be anything that touches the installed driver.

I was told it could be a connectivity issue. The case has been cleaned inside and outside numerous times, hardware were disconnected and reconnected with power connectors and everything, yet the issue wasn't fixed.

My final, working fix for this issue is installing the mentioned 24.8.1 driver, then it never ever happens. Every single version after that seems to have this issue for me.

The driver I'm using is getting pretty old at this point and I'm sure its degrading performance for newer games but sadly I cannot update yet or the system becomes unstable.
Looking for advice or fix with this issue
