Your system (OS) or a driver you have installed on your system or an application that you run stops responding, and possibly the graphics card driver assumes an error in the system and causes this error. So the problem seems that the graphics driver is not causing the error (because fault modules unknown). Graphics driver senses there is an error due to delays (lags) and behaves like there is an error.
You can check which drivers on your system cause delays with LatencyMon software.
CPU-OC or RAM-OC can cause these problems. You can try to reset the BIOS.
With clean Windows installation, you can only try to install the graphics driver (without other drivers).
