  I don't know why but this always happens i haven't even been updating the software but everytime things start to go well this happens and im no longer able to use it to keep track of things with my games

Do a full install when you upgrade, so it updates both the driver and the Radeon software at the same time.

how do i do a full install?

I would uninstall the previous installation with https://www.amd.com/en/support/kb/faq/gpu-601 

then download latest driver https://www.amd.com/en/support and do a full install (not drivers only)
