You cannot hook up 3 monitors to eyefinity without using display port. The HDMI and single link DVI port share the same single and cannot be used at the same time. You must use an active display port or mini display port to single link DVI adapter to connect the 3rd display. They are about $25. It MUST be active. Not passiveOn a side note this has been answered hundreds if times on this forum, AMD tells you this all over the eyefinity Page, and google would have found out in seconds. In the graphics card, there are normally two DVI ports. One of the DVI ports is the DVI-S which shares the clock with the HDMI port & only one of them can work at the same time. Perhaps you used this DVI-S port.
Try to connect your 21" to the "other" DVI port.

ok, was just making sure, as far as i know you cant use the HDMI and the 2 DVI's because one of the DVI's is on the same link as the HDMI.
Why the DP-DVI adapter doesnt work with the 2 other DVI's im really not sure, heres a list of the recommended adapters - http://support.amd.com/us/eyefinity/Pages/eyefinity-dongles.aspx
You cant go above 1920x1200 resolution on 1 screen with a passive adapter but theyre a lot cheaper.
Im using 2 DVI's and an active adapter for the DP->DVI without any issue( just pointing out that its pretty much the same as yours apart from the adapter ).
Can you try it like this:
1 HDMI
1 DVI
1 miniport->DVI
if that doesnt work
swap the DVI to user the other DVI
You could be connecting the DVI & HDMI that use the same link so swapping to the other DVI should work.
