Summary of issue:
High fps in games with 7900 XTX but feels laggy/not smooth.
What I have tried:
I have reinstalled drivers with DDU and have even tried a combination of anti-lag and free-sync with no luck. I put my old GTX 1080 inside my new system and have found my gameplay to be perfectly smooth, which leads my to believe that this has nothing to do with the cable or monitor and something with the Adrenaline drivers.
Note: when playing games my 7900 XTX has not spiked passed 70 C where junction temp is also completely fine.
For reference I was testing out Overwatch where my gameplay feels normal on the GTX 1080 but not on the 7900 XTX.

Same thing here, at this point im considering returning the card.
It cant be that a 1200 Euro graphics card wont run any games smooth out of the box.
Even osu! a graphicly simplistic rhythm game lags on my pc everytime when something happens on my second monitor.
Team Red was a big mistake.
I've got the same problem on my 6600 XT for a few days now. The only reliable solution for me is to either unplug my second monitor or to play games in windowed (borderless has the same problem) mode
I turned off enhanced sync for my Rx 6600 and it fixed the problem for me.
Enhanced Sync: Only use this if you unlocked the fps AND the game is running above the monitor refresh rate.
Sync in Adrenaline: Only use this if you locked your fps to your refresh rate AND the game is not running below the minimum frames of your monitors free sync range.
In both cases, don't use the sync via the in game settings.
Do not use BOTH at the same time. If you have two monitors, make sure they are both running same refresh rates.

I felt the same feeling too especially on Enlisted at 1080p 144 hz monitor.
Firstly i tought it was a cable fault and switched it with a new one and disabled V-sync all games and only set frame rate control for 142 fps in my case cause of 144 hz monitor.
Only use Radeon adrenaline with anti lag except Enlisted game i set tesselation from amd optimized to use applications settings and Freesync Premium activated and without V-sync things looks a little better.
For ex this game Enlisted is a game in development yet and last game engine called Dagor or is not optimized yet better or new engine version can bring less performance even decreasing from ultra settings to Medium settings with V-Sync on.
Other example an old game Battlefield 1 a very good and detailed graphic levels and i can play in single and multiplayer in ultra settings with or without v-sync.
Warthunder runs well even with textures in full ultra Hq with graphics in max settings but in this case i use basically planes in this game but i did a test with tanks and frames always running very high between 130-142 fps.
And last one is another old game Assetto Corsa with graphics in max settings and no problems single or multiplayer running smooth at 144 with v-sync or frame rate control at 142 fps.
Enslisted as i said i felt same feeling as you high frames but not smooth but doubt if is a gpu driver fault or problems in servers or even in a better optimization after last big update they did some weeks ago.
Another question is my old monitor 75 Hz and this one a 144 hz that i need any time to understand better what it can do in games because i know i don t have an high end gpu at this moment.
Try not use V-sync and see if things are better for you.
