GPU drivers reported as "unknown" in dxdiag

Hey y'all, a bit of a computer novice and built a rig that's giving me some issues. Specifically hit a breaking point trying to play Hades II early access that just came out - the game crashes upon launch constantly, and I've done everything I can to troubleshoot, eventually sending dxdiag to support who noted: 

 

"Looks like something strange is going on with your GPU drivers as they are reported as "Unknown" in dxdiag:
 
             Driver Name: Unknown
          Driver Version: Unknown
             Driver Date: Unknown"

Happy to share more if helpful, but does anyone know of potential causes or fixes?

 

I've gone to http://support.amd.com/en-us/download and everything is up to date.

More info, potentially related:

I tried running dxdiag again and got an error prompting 
 
"DxDiag has detected that there might have been a problem accessing Direct3D the 
last time this program was used. Would you like to bypass Direct3D this time?"
 
It seems the only way to run dxdiag is by bypassing Direct3D
