I m using ATI Radeon HD 5400 Series and using all 3 ports with 3 lcd monitors, but i get only 2 display simultaneously, how can i get all 3 display simultaneously?

You cannot run enable 3 monitors simultaneously on that card.

More specifically, only 5450 cards with DisplayPort are capable of driving three monitors.  If your card has HDMI instead of DisplayPort, it can only use two outputs at a time.
