RX 560 4GB dual monitors not working

I recently bought the RX 560 4GB to replace my old Nvidia GTX 750 ti. my dual monitor setup worked fine on the old Nvidia card, but not on this one. I have one monitor hooked up on DVI-D and the other on HDMI. When plugged in independently, each monitor works fine on its own, but when I plug both monitors in together, the one on DVI-D stops being detected and won't display anything. The weird thing is that is that every couple of seconds, windows will be able to detect it for a moment, and then it stops detecting again. It's like the monitor is being repeatedly plugged in and then unplugged.

Again, these monitors together worked fine on this computer with my old graphics card, but not on this one.

  • OS: Windows 7 64 bit
  • Monitor 1: Acer v223W (DVI-D)
  • Monitor 2: Insignia NS-32D312NA15 (HDMI)
