AMD Radeon R9 380 Series crashing, multiple problems, windows10 64bit. What needs to be fixed?

Alright guys, here we go. I am very new to computers and i believe quite a bit needs to be fixed with mine. I think i am bottle-necking, just not sure exactly what...

Level 100 noob here, so please be patient with me.. lol.

 

General problems, would like to know if its my graphics card or my other specs. Something is definitely bottle-necking something i believe, and im sure i need to update quite a bit but honestly dont know how :/ i used to run any game i play [csgo, overwatch, starwars battlefront, h1z1, TF2, and many more]on ultra 1920x1080HD

usually everything is pretty high, i play star wars battlefront quite a bit and my radeon relive settings are this

Screen res  -1920x1080

anti aliasing - adaptive multisampling

morphological filter is on

anisotropic filtering - application settings

texture filtering - performance

surfrace formate optimazation - on

vertical refresh - always off

openGL triple buffering - on

shader cache - AMD optimized

tesselation - AMD optimised

frame rate  control - 300
