PC freezes after series of 4104 display warnings "amdkmdap stopped responding" after Windows 10 Creators Update

After installing Windows 10 Creators Update and newest AMD drivers, I constantly get display warning 4101 in event log, while using Spotify, foobar, YouTube, or any video game. After successive warnings, my display turns black, music stops playing, and pc is unresponsive. This requires turning the power off, disconnecting HDMI cable, turning the power on, and reconnecting the cable. This issue is reproducible each time. Additionally, Display Port connection stopped working, resulting in a "no signal detected from source" message from the monitor.

 

As a solution, I've tried uninstalling GPU drivers with DDU, installing older ones, uninstalling Windows update, but none of it worked. Help would be appreciated, as currently, my pc is unusable whatsoever.
