Two Native DP+two HDMI will work on your single RX 480
If you are going to buy identical card and use Crossfire mode, all of your monitors must be connected to the main GPU anyway.
Otherwise you will lose ability to use Crossfire mode or monitors connected to the second card.
Yes that is possible with Eyefinity, it allows you to configure the layout and you can arrange them in 2x2 fashion.
The question is - what GPU and what monitors are you going to combine in this mode?

Are you looking to play games on this Eyefinity setup, or is it work focused as in your screenshot? That GPU can run up to 6 displays (running 6 requires a DisplayPort MST Hub) but for 4 monitrs you can use the 2 DisplayPort and 2 HDMI connections.
If you are planning to only run it at 1080p (FullHD as you say) and game on it at the composite ~4K resolution, then I would suggest a second RX 480 in Crossfire mode to push games at that resolution.

I want to add 2 more 4k freesynch panels in eyefinity with the first... what should I look for in a DisplayPort MST Hub?
Is it possible to maintain 60hz through the MST Hub?
And models of your monitors(or at least their available video inputs)?
I guess that will have to use the second card so it should be no problem with it (other than financial). The monitors are ViewSonic VX2276-SMHD Silver or somethin like this
