- Which driver is installed?
- What are the exact make and model of monitors?
- Are you using 3x identical DP-VGA adapters?
- What is the make and model of the DP-VGA adapters?
- What are you trying to achieve? Clone mode or Extended?
Here is what I found:
The 2 new monitors are PSI LED 22W PLL2210W .
The MDP-VGA adapters are Accell B101B-002B UltraAV
I'm trying to have all 3 monitors extended.
The computer isn't at my location, so I don't know the driver version. I used the CD that came with the card, which was purchased a couple weeks ago.
The user told me that if they force the old monitor (17" 5:4 ratio) to a resolution using EyeFinity, all 3 monitors worked as expected.
The problem is the the resolution isn't correct for the old monitor. They could not identify the make or model of the old monitor (no discernible markings).
SO, right now I think the problem is the old monitor. I only tested with the 2 new monitors here before the system was deployed.
I've ordered a new Planar LCD 997-7244-00 (They need a 17" 5:4 ratio or all 3 monitors won't fit the space) and will try that next.
I'll let you know how it comes out.

We replaced the "old" monitor and everything works. I don't know what but maybe it was malfunctioning or maybe the card couldn't detect the correct resolution.
Thanks everyone for your help.
