I have this error too on my Sapphire Nitro+ RX 470. Reboot PC solves the problem but It's annoying. Also, Wattman is crashing sometimes. I really, really hope AMD releases best drivers for new RX graphics cards...
Sapphire RX470 NItro+ with latest drivers here and I have this same exact issue. On Sleep/wake cycle the GPU clock is @ 1260 while the Memory Clock is @ 300 (idle) I have tried uninstalling and reinstalling the drivers twice now. I've also experienced black screen and restarts Which I've never experienced before with any AMD drivers.
I keep checking the amd driver page each day for a update.. /cross fingers.
Also, restarting my PC did fix this issue, for the day at least.
I've already sent in a report on this, but I would like to add that for me putting my PC into hibernate does NOT stop this from happening. Shut down, Sleep, Hibernate will do this, only a restart corrects it.
Simple way to get around this for now is to just push the reset button on your pc. Once it restarts the problem will be gone. Download GPU-Z and run it as soon as your pc starts to see if it's maxing out. If it is maxed out reset the pc.
Why GPU-Z and not open Wattman? Because Wattman will hang when opening while the GPU is maxed out. Gpu-z wont. Also, DO NOT LET THE COMPUTER GO TO SLEEP. If it goes to sleep and you wake it up, you will see maxed out gpu. Hang tight, they're working on new driver updates. Remember, this is new tech on new software so sometimes things will happen.

Here's some new information.
This card has been moved into a different PC, with virtually identical hardware.
Now the GPU is running normally, but the RAM is always running at it's maximum speed, and the fan continues to not turn on ever.
I'm having a similar problem as described here: RX 460 - GPU fan doesn't work! How do I fix it?
Exact same card, exact same problem (fan always at 0RPM) except I don't get GPU at 100%.
What motherboard are you using?
This may or may not help all of you with GPU fan problems. I have the GPU working normally in a computer that isn't going to sleep. (Minus the fact that it wants to run the RAM at 100% ALWAYS with multiple monitors)
I am using the Sapphire software to manually engage the fan. The AMD drivers WILL NOT DO IT.

bcrsean, are you using the PowerColor Red Dragon card or the Sapphire card?
I think I've found a workaround to this issue. This appears to be an issue with the latest 16.8.2 as I did not experience it on 16.7.1.
Go to Radeon Settings -> Gaming -> Global Settings -> Power Efficiency
Turn 'Power Efficiency' on. Then disable hybrid sleep. Put the computer to sleep, wake from sleep and then check GPU-Z or Wattman to see that the GPU Core Clock has returned to 300Mhz.
The issue seems to occur when waking from sleep after the computer has been asleep for several hours.
