The card was working fine before the latest update, attempts to roll back the driver/uninstalling-reinstall have failed to correct the issue.
The AMD clean uninstall utility was used.
There are no "ghost" cards which are still installed, yet the drivers listed for the card are in duplicate (unsure if this is intended or not).
"Failed to create OpenGL context for format QSurfaceFormat(version2.0,
options QFlags(), depthBufferSize24, redBufferSize -1, greenBufferSize -1,
blueBufferSize -1, alphaBufferSize 8, stencilBufferSize 8, samples -1,
swapBehavior 2, swapInterval 1, profile 0).
This is most likely caused by not having the necessary graphics drivers installed.
Install a driver providing OpenGL 2.0 or higher, or, if this is not possible,
make sure the ANGLE Open GL ES 2.0 emulation libraries (libEGL.dll, libGLESv2.dll,
and d3dconpiler_*.dll) are available in the applications executable's directory or in
a location listed in PATH."
This notification just popped up, I'm unsure if it is related.

Please try the following steps to restore an older driver to see if that resolves the issue.
1. Use DDU to sweep the system of AMD drivers in safe mode, then restart.
2. Install the latest Catalyst 15.11.1 Beta drivers once the above is completed and test to see if the issue is resolved.
If the issue persists, please provide more information, including how the GPU is connected to the display.
