R9 390 (only a month old) - Screen freezes, pixels appears, mouse and keyboard stops working (see pic)

All of my drivers are up to date (motherboard, gpu, cpu)

The only program I was running at the time was Firefox. I also tried turning hardware acceleration off.

This is happening multiple times a day.

I am 90% this GPU is defective. If so, what steps do I need to take to get it replace/RMA?

Storage - 111GB KINGSTON SV300S37A120G (SSD) 26 °C 931GB Seagate ST1000DM003-1ER162 (SATA) 27 °C

Optical Drives - No optical disk drives detected

Audio - AMD High Definition Audio Device
