Can you run TechPower GPU-Z to get the exact model of your card? Is this a Laptop and Laptop Model?

If you are using a Laptop and it is using an Intel APU then you can download the driver for your HD76xxM GPU card from here: https://www.amd.com/en/support 

All the HD 76xxM Drivers are the same because your GPU card is considered to be legacy and not supported by AMD with anymore Driver Updates.

So you have a choice of two last driver updates:

1- WHQL - 2015

2- BETA - 2016.

BUT if you have a laptop with an AMD APU you must download the AMD APU Driver which will install automatically the HD76xxM GPU driver.
