I have been getting Various Graphical Issues lately that suddenly started happening today.
These include texture/model warping in games and graphical flickering in videos on youtube.
I have provided image examples.

I had these when playing Halo series, not sure why. Turning off FreeSync worked out for me.

I've tried to disable freesync both on my monitor and in the app, so far no luck, 

I have reinstalled windows in an attempt to nullify any incompatible programs or apps. 

I haven't noticed anything yet.

What is the average/maximum temps on that GPU? What GPU is it?

Also, do an OCCT GPU Stress test or 3Dmark, Heaven and check for issues, if they occur you should try another GPU.

Maybe using another PCIe slot just to, for troubleshooting. Doesn't hurt to try.

I have located the issue and its a simple one.

the gpu is failing.
