ASUS Radeon RX470 4 GB ROG Strix. Crash in games

Good afternoon dear support. There is a game GTA V license. Settings Ultra\High. Frequent driver failure on a brand new video card Asus Radeon RX470 O4G - after half an hour of game monitor goes out, nothing responds, even the reset button on the computer (the computer restarts, but the driver does not start working), only after a complete shutdown of the computer screen starts to show.

It is very annoying when in the midst of the game does not show anything, and the sound is heard. After reboot in the event log there is an entry "display Driver amdkmdap stopped responding and has successfully recovered."

Also crashes in the game Witcher 3, but can not set the frequency of departures.

In the game CS:GO departure occurs in "Classic competitive", however in "classic sasual" and "Deathmatch" departures are not observed.

Bottom attached log, which ends in the departure of the last line in CS:GO.
Underneath log "Heaven Benchmark 4.0".

Help to deal with the problem or tell me exactly whether it is a video card defect.

What else do you need to attach logs for a more detailed description of the problem?

Solve the problem. The card was disconnected due to overheating, after installing 6 coolers on the body, the card stopped disconnecting.
1 fan - in front
2 and 3 fans - blowing on the side cover
4 and 5 - blowing from above over CPU cooler
6 fan - blowing as a continuation of the flow from the processor

Fans separateline 3 through the fan controller

Now in the games the temperature does not rise above 62C
