AMD Radeon 295x2 GPU possibly causing PC to freeze to black screen.

For the past few months my PC has been inconsistently freezing and going to a blank screen. All fans then go full blast. Audio cuts out a few moments afterwards and the only way to continue is to cold shut down the computer. I've made sure to update my OS (Win10) and graphics drivers to the latest versions (using Radeon Adrenalin). Sometimes this issue doesn't occur for multiple hours, but other times it occurs only moments after booting. Unplugging the GPU and using the onboard graphics seems to remedy this issue for the time being. I've even updated to a newer power supply, so that doesn't seem to be an issue.
System Info:
Mother Board: ASUSTeK COMPUTER INC. MAXIMUS VII HERO

RAM: 16.0GB Dual-Channel DDR3 @ 666MHz
CPU: Intel Core i7 4790K @ 4.00GHz Haswell 22nm Technology

The issue has been occurring more and more frequently shortly after and even before BiOs finishes booting, however due to the inconsistent nature of this problem, I'm not sure if this is entirely a hardware issue where I may need to replace the GPU. If a replacement is necessary, I would appreciate recommendations!
