GPU = Broken 

How and why? 

notes:

• AMD RX 460 4GB 
• had it for just over 3 years now and used it pretty much all day, been having many issues with it since say 1
• no water or damage like that has been on it, near it just dust
• Always clean every month

Another thing, the attached image is what i saw, its happened many times but 9/10 it would go back to whatever i was doing beforehand, this time not, had to restart my PC.
