Crashing, black screen - no signal D3D error

Playing The Witcher 3 causes my the screen to go black (with no signal) and I have to restart or sometimes it crashes into Desktop, my system is up to date alongside with drivers, no mods no overclocking or anything everything is stock. I've also tried every option in Adrenalin software as well as DDU no luck. Please see attached DxDiag and game crash log. I'm also attaching crash screen from Life is Strange game. (Btw game like League of Legends works fine) I see a lot of people complaining about similar problems I would really appreciate a response from AMD acknowledging a problem.

Computer Type: Desktop - custom build.
GPU: PowerColor AMD Radeon RX 5700 XT 8GB GDDR6 (stock).
CPU: Ryzen 5 3600 (stock).
Motherboard: MSI x470 Gaming Plus.
RAM: 16GB (2x8GB) Corsair DDR4 Vengeance LPX Black, PC4-21300 (2666) (stock).
PSU: OCZ OCZ-ZS750W-UK ZS Series 750W 80+ Bronze +12V Rail.
Operating System & Version: Windows 10 Pro (fully updated).
GPU Drivers: Radeon software 19.7.3.
Chipset Drivers: v1.07.07.0725.

I have this issue as well. 

Playing on highest settings, with enhanced from AMD adranalin drivers (with profile). Switching the profile on/off does not change anything.

In Heart of Stone expansion during the scenes for a marriage quest I get an OpenGL error (see picture) radeon open GL issue.PNG

stating to update the drivers. I am running the most recent drivers...
