AMD VII Blue Screen Crash

On any graphics setting (low to ultra) Graphic intensive games cause a blue screen of death citing memory issues for the GPU. I've never submitted to a form so please let me know what info needs to be provided to get proper advice, thanks all! (I've combed this forum for a similar issue and have been unable. If there is a similar post I'll happily take a link.) 

 

Driver Version: 20.11.2 (Issue can be recreated on older drivers, I have tested).

Issue Expanded: 

To reproduce the BSOD, I play COD Black Ops on any graphics setting. The first round of multiplayer goes fine, but when loading into a second match, upon the loading screen the system Blue screens and crashes. Upon automatic restart the image on screen is fuzz, black and white and all around very messed up.
