Sounds like your GPU card may be going bad. Download a free diagnostic program called OCCT and run the GPU Test with Errors box checkmarked for an hour on full screen. See what it indicates. If it passes the test, Try installing your HD5570 in another computer and see if it does the same thing.
Also your HD5570 is considered to be obsolete and not supported by AMD Driver updates. The last driver by AMD Support you can download from here : Legacy .
Also post the Make & Model of your PSU and Wattage besides the rest of the information required by AMD Forum.
